Fees and Insurance

We participate in most insurance plans including Personal Choice, Medicare, Blue Shield, and many HMO plans. If your insurance plan requires you to have a referral to see a specialist, it must be properly dated and marked for specific procedures. You will not be able to use your HMO insurance without obtaining a correctly completed referral prior to your visit.

Co-payments are due at the time of service and may be paid by cash or check. A $10 administrative fee will be charged , if your co-payment is not paid at the time of service.

Deductibles and co-insurance are the patient's responsibility. You will be billed for these fees after your insurance has paid for the services you received.
